This report will show the ARID, Profile Number, Creation Date, Social Security Number, Medical Record Number, Date of Birth, Last Name, First Name, Middle Name, Full Name and the Guarantor Profile Number. This information may be used to identify duplicate profiles.

 

 

Profile Report - Demographics Information

 

 

 

Listed below is an explanation of each column.

 

•ARID (Accounts Receivable Identification): This will display the id number of the facility

 

•Profile# (Profile Number): Pulls from the Person Profile# field on the Person Profile

 

•Creation Dt (Creation Date): Pulls from the Date Created field on the Person Profile

 

•SSN (Social Security Number): Pulls from the Social Security field on the Person Profile

 

•MR# (Medical Record Number): Pulls from the MedRec# field on the Person Profile

 

•DOB (Date of Birth): Pulls from the Birth Date field on the Person Profile

 

•Last Name:Pulls from the Last Name field on the Person Profile

 

•First Name:Pulls from the First Name field on the Person Profile

 

•Middle Name:Pulls from the Mid Name field on the Person Profile

 

•Full Name: Pulls from the Full Name field on the Person Profile

 

•Guar Profile# (Guarantor Profile Number): Pulls from the Social Security field on the Guarantor's Person Profile
