Forms Grouping

Forms Grouping enables administrators to organize forms into logical categories for easier management. By default, all forms start in an Ungrouped state. Grouping is optional and intended solely to provide a structured way to categorize forms for easier management. Groups will be visible when manually requesting forms and during the form selection step in automated workflows. Regardless of grouping, form attributes must be configured before the form can be used in an automation track.

 

NOTE: Users must have Patient Connect Admin rights in order to create forms groupings.

 

 

Creating Forms Groupings & Adding Forms

 

To create a forms group, select Create Form Group from the Forms Groupings section. Enter a Form Group Name and select Save.

 

NOTE: If creating a Forms Grouping, the attributes are required to be configured.

 

To add forms to the group, expand the grouping and select Add Form.

 

Each form must be configured with appropriate attributes to ensure proper categorization and automation behavior. The required attributes for each form are as follows:

Form: The form to be added to the grouping. Only forms not assigned to a grouping will be available.

Image Title: This field will populate with the Image Title associated with the selected form.

Gender: The gender the form applies to. Options are N/A, Male Only, or Female Only.

Interval: When the form should be requested. Options are After Appointment (Next Day), At Check-In, Calendar Year (Jan. 1 - Dec. 31), Every Appointment, Monthly, Once Only, or Yearly (within the given calendar year).

oNOTE: The After Appointment option will be invoked if an appointment has a status of Scheduled, Checked-In, or Complete. It will not send if marked as Canceled or No Show.

Age: The age range the form applies to. The range may be anywhere from 0 to 125.

 

Select Save once all selections have been made.

 

Continue this process until all forms have been assigned to a grouping.

 

If any of the attributes need to be changed on a form within a grouping, select the edit_icon_2Edit option next to the form. If a form needs to be moved to a different grouping, select the move_iconMove option and then select the grouping to move the form to.
