The CCDA Migration Exception Log captures clinical reconciliation exceptions when they are generated from the migration tool.
CCDA Migration Exception Log
Listed below is an explanation of each row:
•Header:
▪CCDA Success/Fail: Displays the total successful and failed records respectively for the entire report in a #/# format.
•First Row:
▪Date/Time: Displays the date and time of the exception
▪Medical Record#
▪Profile#
▪Patient Name
▪DOB: Displays the patient's date of birth
▪Gender: (M)ale or (F)emale
▪CCDA Patient ID
•Second Row:
▪Meds Success/Fail: Displays the total successful and failed medication records respectively for the patient in a #/# format.
▪Problems Success/Fail: Displays the total successful and failed physician problem records respectively for the patient in a #/# format.
▪Imm Success/Fail: Displays the total successful and failed immunization records respectively for the patient in a #/# format.
▪Allergies Success/Fail: Displays the total successful and failed allergy records respectively for the patient in a #/# format.
