The CCDA Migration Exception Log captures clinical reconciliation exceptions when they are generated from the migration tool.

Listed below is an explanation of each row:

 

Header:

CCDA Success/Fail: Displays the total successful and failed records respectively for the entire report in a #/# format.

 

First Row:

Date/Time: Displays the date and time of the exception

Medical Record#

Profile#

Patient Name

DOB: Displays the patient's date of birth

Gender: (M)ale or (F)emale

CCDA Patient ID

 

Second Row:

Meds Success/Fail: Displays the total successful and failed medication records respectively for the patient in a #/# format.

Problems Success/Fail: Displays the total successful and failed physician problem records respectively for the patient in a #/# format.

Imm Success/Fail: Displays the total successful and failed immunization records respectively for the patient in a #/# format.

Allergies Success/Fail: Displays the total successful and failed allergy records respectively for the patient in a #/# format.
