"Add to Request" Button in Chart Communication -- SCU-10654

DESCRIPTION: An "Add to Request" button has been added to the action bar on the Chart Communication screen. Users can select an existing communication within the list and then select this button to add a message within the thread of the existing communication. This serves the same function as selecting the "Add Request" button while viewing an existing communication, but allows users to add to the communication thread from the list rather than needing to open the communication first.

In addition, when Chart Communication threads are created, the number of messages in the thread now appears in parentheses to the left of the message subject within the list.
