The ED Readmissions Dashboard tracks emergency department readmissions occurring within 24 and 72 hours of a prior visit. It provides detailed breakdowns for each time frame, including Readmission Totals, Visits by Attending Physician, Visits by Readmitting Physician. This data supports the identification of readmission patterns and opportunities for improving care continuity and patient outcomes.

ED Reasmissions

 

 

Metrics

 

•24 Hours ED Readmission: The number of patients who have an ED visit Admit Date and Time within 24 hours of a previous ED visit's Discharge Date and Time.
 

•72 Hours ED Readmission: The number of patients who have and ED visit Admit Date and Time within 72 hours of a previous ED visit's Discharge Date and Time.

 

NOTE: To be considered a readmission, the visits must have different Visit IDs under the same Profile number.

 

 

Filters

 

Filters can be applied to refine the dashlet totals. Each filter features a drop-down menu with a built-in search field, allowing for efficient selection of specific criteria.  

 

•Appointment Date: Use the calendar or slider to specify an Appointment Date range to be used.

 

•Facility: Use the drop-down menu to select a specific facility.
 

•Attending Physician: Use the drop-down menu to select a specific attending physician to be used.
 

•Readmitting Physician: Use the drop-down menu to select a specific readmitting physician to be used.
 

•Clear All Filters: Select this option to clear all previously applied filters.

 

NOTE: If no filters are selected, the data will populate using the default setting of All.
