The Referrals and Transitions of Care Dashboard provides insights into the Internal and External Referrals and Transitions within the facility by providing totals for the number of referrals and transitions, inbound and outbound transitions of care, and inbound and outbound referrals. Additionally, inbound and outbound data can be analyzed by provider, reason, status, stay type, sub type, service code, admitting diagnosis, and department.

Referrals and Transitions of Care

 

 

Metrics

 

•Referrals and Transitions: The sum of Inbound Referrals, Outbound Referrals, Inbound Transitions of Care, Outbound Transitions of Care, and Self Referrals.
 

•Inbound Transitions of Care: The number of entries in which the Referral Type is "Referral - Inbound".
 

•Outbound Transitions of Care: The number of entries in which the Referral Type is "Referral - Outbound".

 

•Self-Referrals: The number of entries in which the Referral Type is "Self Referral".

 

•Inbound Referrals: The number of entries in which the Referral Type is "Referral - Inbound".
 

•Outbound Referrals: The number of entries in which the Referral Type is "Referral - Outbound".

 

 

Filters

 

Filters can be applied to refine the dashlet totals. Each filter features a drop-down menu with a built-in search field, allowing for efficient selection of specific criteria.  

 

•Entry Date: Use the calendar or slider to specify an Appointment Date range to be used.

 

•Facility: Use the drop-down menu to select a specific facility.

 

•Status: Use the drop-down menu to select a specific status.

 

•Department: Use the drop-down menu to select a specific department.

 

•Reason: Use the drop-down menu to select a specific reason.

 

•Admitting Diagnosis: Use the drop-down menu to select a specific admitting diagnosis.

 

•Sub Type: Use the drop-down menu to select a specific sub type.

 

•Service Code: Use the drop-down menu to select a specific service code.
 

•Clear All Filters: Select this option to clear all previously applied filters.

 

NOTE: If no filters are selected, the data will populate using the default setting of All.
