Finalizing the Clinical Information Reconciliation

Select Reconcile.

 

If reconciling multiple applications (Allergies, Home Medications, and Problems), the next application's reconciliation screen will display. Repeat steps 2-6 for each type/application.

 

If only reconciling one application or if it was the last of multiple clinical reconciliations, users are returned back to the Clinical Information Reconciliation screen

 

NOTE: When reconciling a medication where the RxNorm or NDC code does not exist in the Micromedex database, a custom home medication will be created in all CAPS.

 

After the clinical reconciliation is performed, the corresponding applications are updated with the reconciled data and their Clinical Information Reconciliation status is set to "Complete".

 

NOTE: After performing a Clinical Reconciliation for imported medications, although not displayed in Clinical Information Reconciliation, pharmacy data regarding the last pharmacy that filled the prescription will be imported through Clinical Information Reconciliation and will populate in the Pharmacy drop-down field and the prescriber's name will populate the Physician field in the patient's Medication Reconciliation and Prescription Entry applications. Furthermore, any information in the SIG/Directions line will also be imported through Clinical Information Reconciliation and will populate the Prescription Detail field in the Home Medication Edit screen in Medication Reconciliation. Please see the Medication Reconciliation User Guide and the Prescription Entry User Guide for more information regarding these applications.

 

NOTE: Users may continue to import additional records for any of the applications when the status is set to complete.
