Selecting Review when an existing medication from a previous visit is to be removed will display the Discontinue Reasons screen prompt.

NOTE: The system will pull forward discontinued medications to a new visit (patient was re-admitted) if the medication(s) has been discontinued within 24 hours.

The description of the medication to be removed will display along with discontinue date and time fields. Discontinue Date and Time may be modified as needed.

Select a DC Reason for the medication, then select Remove. If the desired reason is not listed, double-clicking the <Other> option will prompt users with an Input Prompt screen with the Enter DC Reasons free-text field. After entering a DC Reason, select OK to continue.

Selecting Remove before selecting a reason will prompt users with the Reason for Removal screen.

A warning will display at the bottom stating, "You are removing an existing Prescription. This medication will not display or print to the Discharge Instruction Report."

The medication description will be displayed along with a Reason free-text field.

After entering a removal reason, select Update to continue.
