Forms Automation

Forms Automation enables dynamic form requests based on appointment criteria, automatically sending required forms to patients through their preferred communication method.

 

Forms automation will automatically trigger when a patient confirms their appointment via a bot message or when the appointment is manually confirmed by scheduling staff. Forms automation will also be invoked when manually requesting forms from the forms icon on the appointment tracking board and will automatically select forms based on the defined automation tracks in Forms Configuration.

 

If an automation track is identified, Patient Connect will compare the forms to be requested to those already on file for the patient. Using the attributes given to each form, the system will then check to see if the form is gender appropriate, age appropriate, and needed based on the request interval defined. If forms are needed, the patient is sent a message via their preferred method of communication requesting that the needed information be returned.
